5 tips to keep your dog happy when indoors

<p>The cooler months are well and truly here and the dreary weather is enough to make anyone a little sad, including our furry friends. According to a study by veterinary charity People’s Dispensary for Sick Animals, one in three dogs experience a downturn in mood during winter months. If the rain is preventing you getting out and about, here are five tips to keep your pooch happy and healthy when indoors.</p> <p><strong>1. Stair work/treadmill/indoor pool</strong></p> <p>Use what you have in your home. If you live in a multi-storey place, playing fetch up or down the stairs is a fun way for your pooch to get a workout. Alternatively, if you have a treadmill at home, use it to walk your dog on a rainy day. Swimming is also a great physical activity, particularly if dogs have joint problems.</p> <p><strong>2. Obedience training</strong></p> <p>Dust off the training books and work with your pooch to improve their obedience skills. It will keep your furry friend mentally active and dispel any boredom.</p> <p><strong>3. Hide and seek</strong></p> <p>Dogs need their senses stimulated – it’s why when they’re outside they will listen, sniff and dig out anything that’s out of the ordinary. Keep your furry friend entertained with a game of hide and seek. Place healthy treats around the house to get your pooch curious and exploring old surrounds.</p> <p><strong>4. Rotation diet</strong></p> <p>Rotating proteins (meats, fish, and poultry) and mixing in different forms of food (wet, dry and raw) will keep your dog interested in food and eating. Consult your vet about the type of diet your dog should be on for optimal health.</p> <p><strong>5. Play time</strong></p> <p>Interactive toys are a great way to pass time, stimulate and entertain your pooch inside. What happens when I stop taking a drug like Ozempic or Mounjaro?

<p><em><a href="https://theconversation.com/profiles/natasha-yates-1213624">Natasha Yates</a>, <a href="https://theconversation.com/institutions/bond-university-863">Bond University</a></em></p> <p>Drugs like Ozempic are very <a href="https://dom-pubs.pericles-prod.literatumonline.com/doi/10.1111/dom.12932">effective</a> at helping most people who take them lose weight. Semaglutide (sold as Wegovy and Ozempic) and tirzepatide (sold as Zepbound and Mounjaro) are the most well known in the class of drugs that mimic hormones to reduce feelings of hunger.</p> <p>But does weight come back when you stop using it?</p> <p>The short answer is yes. Stopping <a href="https://jamanetwork.com/journals/jama/fullarticle/2812936">tirzepatide</a> and <a href="https://doi.org/10.1111/dom.14725">semaglutide</a> will result in weight regain in most people.</p> <p>So are these medications simply another (expensive) form of yo-yo dieting? Let’s look at what the evidence shows so far.</p> <h2>It’s a long-term treatment, not a short course</h2> <p>If you have a bacterial infection, antibiotics will help your body fight off the germs causing your illness. You take the full course of medication, and the infection is gone.</p> <p>For obesity, taking tirzepatide or semaglutide can help your body get rid of fat. However it doesn’t fix the reasons you gained weight in the first place because obesity is a chronic, complex condition. When you stop the medications, the weight returns.</p> <p>Perhaps a more useful comparison is with high blood pressure, also known as hypertension. Treatment for hypertension is lifelong. It’s the same with obesity. Medications work, but only while you are taking them. (Though obesity is more complicated than hypertension, as many different factors both cause and perpetuate it.)</p> <p>Therefore, several concurrent approaches are needed; taking medication can be an important part of effective management but on its own, it’s often insufficient. And in an unwanted knock-on effect, stopping medication can undermine other strategies to lose weight, like eating less.</p> <h2>Why do people stop?</h2> <p>Research trials show anywhere from <a href="https://asean-endocrinejournal.org/index.php/JAFES/article/view/1771">6%</a> to <a href="https://pubmed.ncbi.nlm.nih.gov/35015037/">13.5%</a> of participants stop taking these drugs, primarily because of <a href="https://www.health.harvard.edu/staying-healthy/glp-1-diabetes-and-weight-loss-drug-side-effects-ozempic-face-and-more">side effects</a>.</p> <p>But these studies don’t account for those forced to stop because of cost or <a href="https://www.tga.gov.au/safety/shortages/information-about-major-medicine-shortages/about-ozempic-semaglutide-shortage-2022-and-2023">widespread supply issues</a>. We don’t know how many people have needed to stop this medication over the past few years for these reasons.</p> <p>Understanding what stopping does to the body is therefore important.</p> <h2>So what happens when you stop?</h2> <p>When you stop using tirzepatide or semaglutide, it takes several days (or even a couple of weeks) to <a href="https://pubmed.ncbi.nlm.nih.gov/30565096/">move out of your system</a>. As it does, a number of things happen:</p> <ul> <li>you start feeling hungry again, because both <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4119845/">your brain and your gut</a> no longer have the medication working to make you feel full</li> </ul> <ul> <li> <p>blood sugars increase, because the medication is no longer acting on the pancreas to help control this. If you have diabetes as well as obesity you may need to take other medications to keep these in an acceptable range. Whether you have diabetes or not, you may need to eat foods with a <a href="https://www.betterhealth.vic.gov.au/health/healthyliving/carbohydrates-and-the-glycaemic-index">low glycemic index</a> to stabilise your blood sugars</p> </li> <li> <p>over the longer term, most people experience a return to their <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10092593/">previous blood pressure and cholesterol levels</a>, as the weight comes back</p> </li> <li> <p>weight regain will mostly be in the form of fat, because it will be gained faster than skeletal muscle.</p> </li> </ul> <p>While you were on the medication, you will have lost <a href="https://academic.oup.com/jes/article/5/Supplement_1/A16/6240360">proportionally less skeletal muscle than fat</a>, muscle loss is inevitable when you lose weight, no matter whether you use medications or not. The problem is, when you stop the medication, your body preferentially puts on fat.</p> <h2>Is stopping and starting the medications a problem?</h2> <p>People whose weight fluctuates with tirzepatide or semaglutide may experience some of the downsides of <a href="https://pubmed.ncbi.nlm.nih.gov/21829159/">yo-yo dieting</a>.</p> <p>When you keep going on and off diets, it’s like a rollercoaster ride for your body. Each time you regain weight, your body has to <a href="https://www.jomes.org/journal/view.html?doi=10.7570/jomes.2017.26.4.237">deal with</a> spikes in blood pressure, heart rate, and how your body handles sugars and fats. This can <a href="https://cardiab.biomedcentral.com/articles/10.1186/s12933-022-01735-x">stress</a> your heart and overall cardiovascular system, as it has to respond to greater fluctuations than usual.</p> <p>Interestingly, the risk to the body from weight fluctuations is greater for people who are <a href="https://jech.bmj.com/content/74/8/662">not obese</a>. This should be a caution to those who are not obese but still using tirzepatide or semaglutide to try to lose unwanted weight.</p> <h2>How can you avoid gaining weight when you stop?</h2> <p>Fear of regaining weight when stopping these medications is valid, and needs to be addressed directly. As obesity has many causes and perpetuating factors, many evidence-based approaches are needed to reduce weight regain. This might include:</p> <ul> <li> <p>getting quality <a href="https://www.hindawi.com/journals/ije/2010/270832/">sleep</a></p> </li> <li> <p>exercising in a way that builds and maintains muscle. While on the medication, you will <a href="https://pubmed.ncbi.nlm.nih.gov/32628589/">likely have lost muscle</a> as well as fat, although this is not inevitable, especially if you <a href="https://www.europeanreview.org/article/34169">exercise regularly</a> while taking it</p> </li> </ul> <ul> <li> <p>addressing emotional and cultural aspects of life that contribute to over-eating and/or eating unhealthy foods, and how you view your body. Stigma and shame around body shape and size is not cured by taking this medication. Even if you have a healthy relationship with food, we live in a culture that is <a href="https://ajph.aphapublications.org/doi/full/10.2105/AJPH.2009.159491">fat-phobic and discriminates</a> against people in larger bodies</p> </li> <li> <p>eating in a healthy way, hopefully continuing with habits that were formed while on the medication. Eating meals that have high nutrition and fibre, for example, and lower overall portion sizes.</p> </li> </ul> <p>Many people will stop taking tirzepatide or semaglutide at some point, given it is expensive and in short supply. When you do, it is important to understand what will happen and what you can do to help avoid the consequences. Silent cancers: here’s what you need to know when there are no obvious symptoms

<p><em><a href="https://theconversation.com/profiles/justin-stebbing-1405462">Justin Stebbing</a>, <a href="https://theconversation.com/institutions/anglia-ruskin-university-1887">Anglia Ruskin University</a></em></p> <p>The recent revelations about the Princess of Wales’s <a href="https://www.bbc.com/news/uk-68640917">cancer diagnosis</a> highlight a crucial aspect of cancer detection – the disease’s sometimes silent nature.</p> <p>Silent cancers are those without noticeable symptoms. They pose a unique challenge in early detection and treatment.</p> <p>Contrary to common perception, cancer does not always announce its presence through overt symptoms or obvious signs. Many people receive a <a href="https://academic.oup.com/clinchem/article-abstract/70/1/179/7283928">cancer diagnosis incidentally</a>, when it’s found during routine medical examinations or investigations for unrelated health concerns – as seems to be the case for both <a href="https://www.wsj.com/health/kate-middleton-catherine-cancer-what-is-preventative-chemotherapy-9625370d">the princess</a> and <a href="https://www.bbc.co.uk/news/health-68171163">King Charles III</a>.</p> <p>While even silent cancers can sometimes be <a href="https://pubmed.ncbi.nlm.nih.gov/22584215/">aggressive and advance rapidly</a>, they can also remain <a href="https://pubmed.ncbi.nlm.nih.gov/20363069/">dormant</a> for years or <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8819710/">even decades</a>. Some <a href="https://ascopubs.org/doi/10.14694/EdBook_AM.2012.32.98">prostate</a>, <a href="https://ascopubs.org/doi/10.14694/EdBook_AM.2012.32.301">breast</a> and <a href="https://www.frontiersin.org/journals/endocrinology/articles/10.3389/fendo.2020.571421/full">thyroid</a> cancers, for example, <a href="https://www.tmlep.com/clinical-learning/2023-01-23-when-did-this-tumour-start-the-need-for-a-gompertzian-understanding-of-tumour-growth-kinetics">often evolve slowly</a> without obvious symptoms or spreading beyond the original area.</p> <p>Research suggests that some of these cancers are <a href="https://www.frontiersin.org/journals/endocrinology/articles/10.3389/fendo.2020.571421/full">overtreated</a>. Sometimes patients are best left alone or treated much more gently, perhaps even without medical intervention, using a <a href="https://www.nejm.org/doi/full/10.1056/nejmoa1311593">“watch and wait”</a> strategy. This approach may be taken with prostate cancer in the elderly, for example.</p> <h2>The importance of early diagnosis</h2> <p>Whatever the cancer, it’s always important to get an early diagnosis though – and for silent cancers, this is obviously a challenge.</p> <p>Some cancer symptoms <a href="https://pubmed.ncbi.nlm.nih.gov/36702593/">can be vague</a> and easily mistaken for benign ailments. Fatigue, unexplained weight loss and persistent pain are among the nonspecific symptoms that may signal an underlying malignancy. But such symptoms can be misinterpreted or easily dismissed, which contributes to delayed diagnosis and treatment.</p> <figure><iframe src="https://www.youtube.com/embed/MGMy6BzBvp0?wmode=transparent&amp;start=0" width="440" height="260" frameborder="0" allowfullscreen="allowfullscreen"></iframe></figure> <p>Fortunately, in many countries including the UK, we have <a href="https://www.england.nhs.uk/cancer/early-diagnosis/screening-and-earlier-diagnosis/">screening</a> tests for diseases like breast or colon cancer, to increase early diagnoses.</p> <p>Early diagnosis is a <a href="https://acsjournals.onlinelibrary.wiley.com/doi/full/10.1002/cncr.32887">key factor</a> for successful cancer treatment. Detecting cancer in its silent phase offers a window of opportunity for early intervention and improved outcomes. The discovery of asymptomatic cancers through diagnostic imaging or screening tests underscores the importance of these proactive healthcare measures.</p> <p>Identifying cancer at an early stage means the disease is confined to its site of origin, smaller and potentially easier to cure. Diagnosing a smaller cancer often means that if an operation is needed, it may be a less invasive surgery. There may also be a <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6825992/">lower chance</a> of needing post-operative preventative chemotherapy, to mop up any residual cells.</p> <p>Colorectal cancer (CRC) is a good example to show the critical importance of screening. Studies show that patients who participate in CRC <a href="https://www.nice.org.uk/guidance/dg56/chapter/1-Recommendations">screening</a>, such as colonoscopies or tests that look for blood in the stool, are more likely to be diagnosed while asymptomatic and have more positive prognoses after treatment. Those diagnosed with CRC after showing symptoms, such as rectal bleeding or changes in bowel habits, tend to have more <a href="https://bmjopengastro.bmj.com/content/4/1/e000146%20">advanced tumors and poorer outcomes</a>.</p> <figure><iframe src="https://www.youtube.com/embed/nA9_Io3LDpA?wmode=transparent&amp;start=0" width="440" height="260" frameborder="0" allowfullscreen="allowfullscreen"></iframe></figure> <p>Public health initiatives aimed at raising awareness about the importance of both cancer screening and symptom recognition play a pivotal role in reducing diagnostic delays. Empowering people to engage in <a href="https://healthcaredelivery.cancer.gov/prevention/#:%7E:text=Cancer%20can%20be%20prevented%20through,they%20are%20more%20easily%20treated.">preventive healthcare measures</a> such as HPV vaccinations and lifestyle changes that decrease risk can facilitate early detection and intervention, potentially altering the trajectory of the disease.</p> <h2>Biomarker discovery</h2> <p>The latest advances in diagnostic technologies, often known as <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8012218/#:%7E:text=During%20biomarker%20discovery%2C%20evaluation%20of,design%20of%20future%20validation%20studies.">“biomarker discovery”</a>, hold promise for improving early detection rates and refining treatment strategies for silent cancers. From <a href="https://www.cancer.gov/publications/dictionaries/cancer-terms/def/molecular-profiling">molecular profiling</a> to <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9922467/">liquid biopsy techniques</a> (blood tests to diagnose cancer), innovative approaches are reshaping the landscape of cancer diagnosis, offering new avenues for personalised and precision medicine.</p> <p>For example, I worked with a team using blood tests to identify cancers in more than <a href="https://www.nature.com/articles/s41388-023-02591-z">1,000 women recalled after screening for mammography</a>. We looked at the DNA that tumour cells release – so-called <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10496721/">cell-free DNA</a> – and also metabolomics (rare markers related to metabolism in the blood). From this information, we found healthy patients, benign disease, pre-cancer and breast cancer. Although there’s increasing awareness and use of this <a href="https://www.sciencedirect.com/science/article/abs/pii/S1769721218307110">approach in Europe</a>, it isn’t standard in the UK.</p> <p>Asymptomatic cancers represent a formidable challenge for patient care. But, by encouraging patients to adopt preventive lifestyles and engage with screenings and tests, asymptomatic cancers don’t have to be a hidden threat to health.<!-- Below is The Conversation's page counter tag. What happens to your liver when you quit alcohol

<p><em><a href="https://theconversation.com/profiles/ashwin-dhanda-1359529">Ashwin Dhanda</a>, <a href="https://theconversation.com/institutions/university-of-plymouth-717">University of Plymouth</a></em></p> <p>According to Greek mythology, Zeus punished Prometheus for giving fire to humans. He chained Prometheus up and set an eagle to feast on his liver. Each night, the liver grew back and each day, the eagle returned for his feast. In reality, can a liver really grow back?</p> <p>The liver is the largest internal organ in the human body. It is needed for hundreds of bodily processes, including breaking down toxins such as alcohol. As it is the first organ to “see” alcohol that has been drunk, it is not surprising that it is the most susceptible to alcohol’s effects. However, other organs, including the brain and heart, can also be damaged by long-term heavy alcohol use.</p> <p>As a liver specialist, I meet people with alcohol-related liver disease every day. It is a <a href="https://britishlivertrust.org.uk/information-and-support/liver-conditions/alcohol-related-liver-disease/">spectrum of disease</a> ranging from laying down of fat in the liver (fatty liver) to scar formation (cirrhosis) and it usually doesn’t cause any symptoms until the very late stages of damage.</p> <p>At first, alcohol makes the liver fatty. This fat causes the liver to become inflamed. In response, it tries to heal itself, producing scar tissue. If this carries on unchecked, the whole liver can become a mesh of scars with small islands of “good” liver in between – cirrhosis.</p> <p>In the late stages of cirrhosis, when the liver fails, people can turn yellow (jaundice), swell with fluid and become sleepy and confused. This is serious and can be fatal.</p> <p>Most people who regularly drink more than the recommended limit of 14 units of alcohol per week (about six pints of normal strength beer [4% ABV] or about six average [175ml] glasses of wine [14% ABV]) will have a fatty liver. Long-term and heavy alcohol use increases the risk of developing <a href="https://www.nhs.uk/conditions/alcohol-related-liver-disease-arld/">scarring and cirrhosis</a>.</p> <h2>Good news</h2> <p>Fortunately, there is good news. In people with fatty liver, after only two to three weeks of giving up alcohol, the liver can heal and looks and functions <a href="https://arcr.niaaa.nih.gov/volume/41/1/natural-recovery-liver-and-other-organs-after-chronic-alcohol-use">as good as new</a>.</p> <p>In people with liver inflammation or mild scarring, even within seven days of giving up alcohol, there are noticeable reductions in liver <a href="https://www.mdpi.com/2072-6643/13/5/1659">fat, inflammation and scarring</a>. Stopping alcohol use for several months lets the liver heal and return to normal.</p> <p>In heavy drinkers with more severe scarring or liver failure, giving up alcohol for several years reduces their chance of <a href="https://www.cghjournal.org/article/S1542-3565(22)01113-2/fulltext">worsening liver failure and death</a>. However, people who drink heavily can be physically dependent on alcohol and stopping suddenly can cause alcohol withdrawal.</p> <p>In its mild form, it causes shaking and sweating. But if severe, it can cause hallucinations, fits and even death. Going “cold turkey” is never recommended for heavy drinkers, who should seek medical advice about how to safely give up alcohol.</p> <h2>Other benefits</h2> <p>Giving up drinking also has positive effects on <a href="https://alcoholchange.org.uk/blog/benefits-of-dry-january-and-when-you-can-expect-to-see-them">sleep, brain function and blood pressure</a>.</p> <p>Avoiding alcohol for long periods also reduces the risk of several types of <a href="https://www.cancerresearchuk.org/about-cancer/causes-of-cancer/alcohol-and-cancer">cancer</a> (including liver, pancreas and colon) and the risk of <a href="https://www.bhf.org.uk/informationsupport/heart-matters-magazine/medical/effects-of-alcohol-on-your-heart">heart disease and stroke</a>.</p> <p>However, alcohol is not the only cause of ill health. Giving it up has many health benefits, but it is not a panacea. It should be seen as part of a healthy lifestyle, including a balanced diet and regular physical exercise.</p> <p>So, to answer the question posed by the myth of Prometheus, the liver has an amazing power to repair itself after it has been damaged. But it cannot grow back as new if it was already severely scarred.</p> <p>If you stop drinking and only have a fatty liver, it can quickly turn back to normal. If you had a scarred liver (cirrhosis) to start with, stopping alcohol will allow some healing and improved function but can’t undo all the damage that has already been done.</p> <p>If you want to look after your liver, drink in moderation and have two to three alcohol-free days each week. 9 beauty hacks for when you’re running late

<p><strong>Spritz on dry shampoo</strong></p> <p>When you don’t have time to wash or blow-dry your hair, dry shampoo is your new best friend. One quick spritz is a shortcut to volume and oil-free hair. Simply spray the dry shampoo directly at the roots and massage in for a clean, tousled look. Beauty expert and blogger, Jeanette Zinno, shares her special trick, “Use a dry shampoo at night to wake up with fresh textured hair. For example, a colour stick the can be used for cheeks, eyes, and lips. “I love it because it’s also very small, so you can take it on-the-go too,” Zinno says.</p> <p><strong>Dry nail polish fast with ice water</strong></p> <p>We’ve all been there – you’re going to an event or getting ready for a date, and you didn’t have time for a manicure. Of course, there’s no such thing as speedily painting your nails, as polish requires ample drying time, except, that is, if you use this brilliant trick from Zinno. </p> <p>“Soak your freshly-painted nails in a bowl of ice water for a minute; the cold will dry them quickly. Make sure you have the bowl ready before you paint your nails so you don’t mess them up!”</p> <p><strong>Skip the foundation</strong></p> <p>Unless your skin has a lot of unevenness, you really don’t need foundation 24/7. When it comes to weight, your ‘diet’ is much more than what you eat

<p>Atkins, keto, palaeo, gluten-free, low-carb, low-fat, high-fat, raw, vegan, vego, pescatarian – phew, that’s a lot of different diets!</p> <div class="copy"> <p>And it’s by no means an exhaustive list.</p> <p>The old adage ‘you are what you eat’ has come to be a mantra for good diet and health. It was originally coined by 19th-century German philosopher Ludwig Feuerbach, himself drawing on commentary by an earlier French gourmand Anthelme Brillat-Savarin.</p> <p>Increasingly, science is finding new connections between diet and our overall picture of health. You may have heard how our gut microbiome acts as a second brain, or that avoiding unprocessed foods can lead to all-cause mortality.</p> <p>But when it comes to many fad diets that promise quick weight loss or improved health, the science can sometimes be skimp. This can change over time as researchers test the influence of diet on general health, weight management and as a medical treatment.</p> <p>The <a href="https://cosmosmagazine.com/health/mediterranean-diet-heart-dementia/">Mediterranean diet</a> is probably closest to the mark as a lifestyle of choice, in terms of overall health, nutrition, and diet science. It emphasises <a href="https://cosmosmagazine.com/health/nutrition/plant-based-diets-could-prevent-type-2-diabetes/">fruit and vegetable</a> consumption, with some wholegrain breads and cereals, legumes, nuts, seeds and fish, with olive oil as a primary fat source.</p> <p>This diet is either explicitly endorsed by many health authorities around the world such as the <a href="https://www.heart.org/en/healthy-living/healthy-eating/eat-smart/nutrition-basics/mediterranean-diet" target="_blank" rel="noreferrer noopener">American Heart Association</a>, the <a href="https://www.racgp.org.au/clinical-resources/clinical-guidelines/handi/handi-interventions/nutrition/mediterranean-diet-for-reducing-cardiovascular-dis" target="_blank" rel="noreferrer noopener">Royal Australian College of General Practitioners</a> as a diet for lowering cardiovascular disease risk, or used as a basis for other recommendations. The World Health Organization also <a href="https://www.who.int/europe/news/item/07-05-2018-fostering-healthier-and-more-sustainable-diets-learning-from-the-mediterranean-and-new-nordic-experience" target="_blank" rel="noreferrer noopener">advises</a> on ways for the Mediterranean and similar New Nordic diets to be implemented as <a href="https://iris.who.int/bitstream/handle/10665/326264/9789289053013-eng.pdf?sequence=3" target="_blank" rel="noreferrer noopener">health policy</a>.</p> <p>But diet might be better considered about more than what goes in one’s mouth.</p> <p>Dr Evangeline Mantzioris, Program Director of the Nutrition and Food Sciences Degree at the University of South Australia, says a truer interpretation of the world extends beyond merely food and drink.</p> <p>“The word diet actually derives from the Greek word <em>diaita</em>, which means the way you choose to live your life,” Mantzioris told the <a href="https://cosmosmagazine.com/tag/debunks/" target="_blank" rel="noreferrer noopener"><em>Debunks</em> podcast.</a></p> <p>“So it’s not just about the food, it’s about the exercise, it’s about the social interaction, it’s about the rest. It’s about the sleep. It’s all of that.”</p> <p>The WHO’s 2019 Health Evidence Network Synthesis Report also acknowledges both social and sleep components of the lifestyle, noting shared eating practices, post-meal siestas and lengthy meal times all contribute to positive health effects.</p> <p>In terms of the nutritional component, Mantzioris notes that adherence to the diet requires not just an uptake of olive oil, but cutting down on less beneficial foods and an active lifestyle.</p> <p>“It’s not just the olive oil, it’s dropping down the meat, it’s mainly a plant food diet, it’s purposeful exercise,” she says.</p> <p>“I’m always a little bit nervous when people just talk about the diet and the food without considering the rest of it.</p> <p>“In the 60s, when the health benefits of the Mediterranean diet were seen […] they were out there harvesting, growing their food, preparing their food, doing all that sort of purposeful exercise in the outdoor environment, often in quite steep terrain. Meg Ryan is back after a "giant break"

<p>Meg Ryan is back and she has spilled it all, ahead of her first rom-com release in nearly 15 years. </p> <p>In an interview with <em>People Magazine,</em> the <em>When Harry Met Sally </em>star revealed the reason why she took a step back from her career. </p> <p>"I took a giant break because I felt like there's just so many other parts of my experience as a human being I wanted to develop," she told the outlet. </p> <p>"It's nice to think of it as a job and not a lifestyle. And that is a great way of navigating it for me."</p> <p>The 61-year-old also shared the inspiration behind her first rom-com <em>What Happens Later, </em>which she directed, wrote and starred in. </p> <p>"It came to me during lockdown," she gushed. </p> <p>"The essence of it is these two people who are stuck together. I just love that idea that we're held in a space, even if it feels conflicted, maybe for reasons that heal them."</p> <p>This is the first rom-com that she has acted in for over a decade, with her last film in that genre being <em>Serious Moonlight</em> back in 2009.</p> <p>In another another conversation with <em>Interview</em> <em>magazine's</em> Carol Burnett, she opened up about the process of making her film. </p> <p>"Truly, the easiest part was acting in it," she told the publication. </p> <p>"I want to direct again just so I can sit in the chair, because I’m sure there’s a lot of things I missed."</p> <p>"I hadn’t done a role in a really long time, but it was fun with David," she added, referring to co-star David Duchovny, known for his role as Fox Mulder in <em>The X Files</em>.</p> <p>"A lot of it was done in two shots. I’m proud of that. I set up everything beforehand so that once we were there, it was just David and I trying to tell the truth."</p> <p>She revealed that the film was assembled together with a very "deliberate" process and a budget of only $3 million. </p> <p>"We had to do it really quickly. A lot of those extras weren’t even ours, they were real people," she said. </p> <p>"We went back in post and made everybody the same palette. All the reasons you might be having night sweats – and when to see a doctor

Later, it’s a sweltering summer evening and you’re finding it hard to sleep, so you kick off the covers.</p> <p>Sweating is a normal part of the body’s cooling system, helping to release heat and maintain optimal body temperature. But regularly waking up during the night, soaked through from excessive sweating is not.</p> <p>Night sweats are <a href="https://www.mayoclinic.org/symptoms/night-sweats/basics/definition/sym-20050768">repeated episodes</a> of excessive or intense sweating at night. They are an unpleasant part of life for many people.</p> <p>Many conditions and factors can trigger night sweats by changing the body’s tightly regulated temperature set point, at which the body attempts to maintain its <a href="https://www.tandfonline.com/doi/full/10.4161/temp.29702">core temperature</a>. Some triggers are harmless (a hot bedroom) or even related to positive lifestyle changes (exercise). Others have an underlying cause like menopause, infection, disease or medication.</p> <h2>Temperature control and sweating</h2> <p>The hypothalamus, located in the brain, is part of the <a href="https://www.hormones-australia.org.au/the-endocrine-system/">endocrine system</a> and the temperature control centre for the body. It contains <a href="https://www.statpearls.com/point-of-care/29920#ref_19631766">temperature sensors</a> that receive information from nerve cells (thermoreceptors) located centrally (in the organs) and peripherally in the skin.</p> <p>Thermoreceptors detect changes in body temperature, sending signals back to the hypothalamus. These <a href="https://www.sciencedirect.com/science/article/pii/S1876034111000256">signals</a> will either activate sweating to cool the body or shivering to warm the body.</p> <h2>Hormones and night sweats</h2> <p>Anyone, regardless of age or gender, can experience night sweats. But women experience night sweats more often than men, largely because menopause and associated changing hormone levels are <a href="https://www.tandfonline.com/doi/abs/10.3109/13697137.2011.608596">a leading cause</a>.</p> <p>Approximately 80% of women experience <a href="https://link.springer.com/article/10.1007/s00737-007-0209-5">hot flashes</a> (also called hot flushes) or night sweats after <a href="https://www.menopause.org.au/hp/information-sheets/what-is-menopause">menopause</a> (when periods have ceased for 12 months) and during <a href="https://www.healthdirect.gov.au/perimenopause">perimenopause</a> (the time leading up to it).</p> <p>While both hot flashes and night sweats produce a feeling of overheating, they are different experiences associated with menopause. Hot flashes occur during the day, are transient episodes of flushing and may involve sweating. Night sweats occur at night and involve an intense period of <a href="https://www.proquest.com/docview/2821423865?accountid=14649">sweating</a>. Changing oestrogen levels are thought to impact norepinephrine and serotonin levels, two neurotransmitters that influence <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6459071/#:%7E:text=%5B21%5D%20Estrogens%20stimulate%20the%20production,norepinephrine%20which%20disturbs%20hypothalamic%20thermostat">temperature regulation</a> in the hypothalamus.</p> <p>Hormones also influence night sweats in men, particularly those with low <a href="https://www.healthdirect.gov.au/testosterone">testosterone</a> levels, known as <a href="https://www.hormones-australia.org.au/endocrine-diseases/hypogonadism/">hypogonadism</a>. Around 38% of men aged 45 years or older have low testosterone <a href="https://www.scielo.br/j/ibju/a/RZqqfTn5tY6BFpV6rp3GMxJ/">levels</a> but it can affect men at any age.</p> <h2>Infections, disease and medications</h2> <p>When fighting infection, our body temperature often <a href="https://europepmc.org/article/nbk/nbk562334">rises</a>. This can stimulate sweating to cool and decrease body <a href="https://www.sciencedirect.com/science/article/pii/S1876034111000256">temperature</a>.</p> <p>Minor infections like the common cold can cause night sweats. They are also a symptom of serious infections such as human immunodeficiency virus (HIV) and diseases such as <a href="https://www.aafp.org/pubs/afp/issues/2020/0101/p34.html">Hodgkin’s</a> and <a href="https://link.springer.com/article/10.2165/00002018-200831020-00002">non-Hodgkin’s lymphoma</a>. However, night sweats are rarely the only symptom present.</p> <p>Medications such as selective serotonin reuptake inhibitors (SSRIs), corticosteroids, thyroid hormone replacement and methadone can cause night sweats. These medications affect parts of the <a href="https://link.springer.com/article/10.2165/00002018-200831020-00002">brain</a> and neurotransmitters that control and stimulate sweating.</p> <p>Regular alcohol (particularly alcohol dependence) and recreational drug use can also <a href="https://link.springer.com/article/10.2165/00002018-200831020-00002">increase the risk</a> of night sweats.</p> <h2>Stress, snoring and strenuous exercise</h2> <p>Night sweats are commonly reported by people with <a href="https://karger.com/spp/article-abstract/26/2/92/295722/Psychological-Sweating-A-Systematic-Review-Focused?redirectedFrom=fulltext">anxiety</a>.</p> <p>Psychological stress activates the body’s fight or flight system releasing neurotransmitters that increase heart rate, respiration, and blood pressure. This causes the body to heat up, at which point it starts sweating to cool the body back down. Night sweats may also increase anxiety, causing more sweating which in turn leads to less sleep and more anxiety.</p> <p>If anxiety causes night sweats and this causes distress, it’s best to get up, move around and engage in a <a href="https://www.calmclinic.com/anxiety/symptoms/night-sweats">calming routine</a>, preferably in a dark or dimly lit room.</p> <p>Night sweats have similarly been connected with sleep disorders like <a href="https://www.healthdirect.gov.au/obstructive-sleep-apnoea">obstructive sleep apnoea</a>, where the airway is repeatedly blocked during sleep and there is loud snoring. About one third of people with obstructive sleep apnoea regularly <a href="https://link.springer.com/article/10.1007/s11325-011-0502-4">experience night sweats</a>. The exact cause is undetermined but research shows it is linked with low blood oxygen levels (<a href="https://link.springer.com/article/10.1007/s11325-022-02701-3">hypoxemia</a>) and/or <a href="https://onlinelibrary.wiley.com/doi/full/10.1111/j.1365-2869.2009.00743.x">high blood pressure</a>.</p> <p>People can experience night sweats after high-intensity workouts. Vigorous exercise can stimulate the thyroid, <a href="https://www.ncbi.nlm.nih.gov/books/NBK500006/#:%7E:text=Thyroid%20hormone%20increases%20the%20basal,respiration%20rate%2C%20and%20body%20temperature">increasing basal metabolic rate</a> and body temperature for up to <a href="https://journals.lww.com/acsm-msse/Fulltext/2011/09000/A_45_Minute_Vigorous_Exercise_Bout_Increases.6.aspx">14 hours post exercise</a>. So night sweats can occur even after a vigorous morning workout.</p> <p>Night sweats can indicate overtraining and/or under-fuelling. If not enough calories are consumed to support the increase in training, blood sugar could drop and you could experience <a href="https://www.aafp.org/pubs/afp/issues/2003/0301/p1019.pdf">hypoglycaemia</a>, which can cause night sweats.</p> <h2>When to seek help and 5 things to try</h2> <p>There are <a href="https://www.aafp.org/pubs/afp/issues/2020/1001/p427.html">numerous</a> health conditions and medications that can cause night sweats and interfere with sleep.</p> <p>If night sweats are regular, distressing, interfere with sleep or are accompanied by symptoms such as fatigue or weight loss (not related to lifestyle or diet changes) talk to a doctor to help determine the cause. They might suggest alternative medications to any you’re taking or recommend tests or investigations.</p> <p>In the meantime, you can try the following ideas:</p> <p><strong>1.</strong> sleep in a cool room and use a fan if needed</p> <p><strong>2.</strong> don’t overdress for bed. Wear breathable cotton or linen pyjamas</p> <p><strong>3.</strong> choose lightweight bedding you can kick off. <p><em><a href="https://theconversation.com/profiles/greig-de-zubicaray-1468234">Greig de Zubicaray</a>, <a href="https://theconversation.com/institutions/queensland-university-of-technology-847">Queensland University of Technology</a></em></p> <p>We’ve all experienced that moment mid-sentence when we just can’t find the word we want to use, even though we’re certain we know it.</p> <p>Why does this universal problem among speakers happen?</p> <p>And when can word-finding difficulties indicate something serious?</p> <p>Everyone will experience an occasional word-finding difficulty, but if they happen very often with a broad range of words, names and numbers, this could be a sign of a neurological disorder.</p> <h2>The steps involved in speaking</h2> <p>Producing spoken words involves several <a href="https://doi.org/10.1093/oxfordhb/9780190672027.013.19">stages of processing</a>.</p> <p>These include:</p> <ol> <li> <p>identifying the intended meaning</p> </li> <li> <p>selecting the right word from the “mental lexicon” (a mental dictionary of the speaker’s vocabulary)</p> </li> <li> <p>retrieving its sound pattern (called its “form”)</p> </li> <li> <p>executing the movements of the speech organs for articulating it.</p> </li> </ol> <p>Word-finding difficulties can potentially arise at each of these stages of processing.</p> <p>When a healthy speaker can’t retrieve a word from their lexicon despite the feeling of knowing it, this is called a “tip-of-the-tongue” phenomenon by language scientists.</p> <p>Often, the frustrated speaker will try to give a bit of information about their intended word’s meaning, “you know, that thing you hit a nail with”, or its spelling, “it starts with an <em>H</em>!”.</p> <p>Tip-of-the-tongue states are relatively common and are a type of speech error that occurs primarily during retrieval of the sound pattern of a word (step three above).</p> <h2>What can affect word finding?</h2> <p>Word-finding difficulties occur at all ages but they do happen more often as we get older. In older adults, they can cause frustration and anxiety about the possibility of developing dementia. But they’re not always a cause for concern.</p> <p>One way researchers investigate word-finding difficulties is to ask people to keep a diary to record how often and in what context they occur. <a href="https://www.frontiersin.org/articles/10.3389/fpsyg.2015.01190/full">Diary studies</a> have shown that some word types, such as names of people and places, concrete nouns (things, such as “dog” or “building”) and abstract nouns (concepts, such as “beauty” or “truth”), are more likely to result in tip-of-the-tongue states compared with verbs and adjectives.</p> <p>Less frequently used words are also more likely to result in tip-of-the-tongue states. It’s thought this is because they have weaker connections between their meanings and their sound patterns than more frequently used words.</p> <p>Laboratory studies have also shown tip-of-the-tongue states are more likely to occur under <a href="https://www.tandfonline.com/doi/full/10.1080/13825585.2019.1641177">socially stressful</a> conditions when speakers are told they are being evaluated, regardless of their age. Many people report having experienced tip-of-the-tongue problems during job interviews.</p> <h2>When could it spell more serious issues?</h2> <p>More frequent failures with a broader range of words, names and numbers are likely to indicate more serious issues.</p> <p>When this happens, language scientists use the terms “anomia” or “<a href="https://www.aphasia.com/aphasia-library/aphasia-types/anomic-aphasia/">anomic aphasia</a>” to describe the condition, which can be associated with brain damage due to stroke, tumours, head injury or dementia such as Alzheimer’s disease.</p> <p>Recently, the actor Bruce Willis’s family <a href="https://edition.cnn.com/2023/02/16/health/frontotemporal-dementia-definition-symptoms-wellness/index.html">revealed</a> he has been diagnosed with a degenerative disorder known as primary progressive aphasia, for which one of the earliest symptoms is word-finding difficulties rather than memory loss.</p> <p>Primary progressive aphasia is typically associated with frontotemporal or Alzheimer’s dementias, although it can be associated with other <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3637977/">pathologies</a>.</p> <p>Anomic aphasia can arise due to problems occurring at different stages of speech production. An assessment by a clinical neuropsychologist or speech pathologist can help clarify which processing stage is affected and how serious the problem might be.</p> <p>For example, if a person is unable to name a picture of a common object such as a hammer, a clinical neuropsychologist or speech pathologist will ask them to describe what the object is used for (the individual might then say “it’s something you hit things with” or “it’s a tool”).</p> <p>If they can’t, they will be asked to gesture or mime how it’s used. They might also be provided with a cue or prompt, such as the first letter (<em>h</em>) or syllable (<em>ham</em>).</p> <p>Most people with anomic aphasia benefit greatly from being prompted, indicating they are mostly experiencing problems with later stages of retrieving word forms and motor aspects of speech.</p> <p>But if they’re unable to describe or mime the object’s use, and cueing does not help, this is likely to indicate an actual loss of word knowledge or meaning. This is typically a sign of a more serious issue such as primary progressive aphasia.</p> <p><a href="https://en.wikipedia.org/wiki/Neuroimaging">Imaging studies</a> in healthy adults and people with anomic aphasia have shown different areas of the brain are responsible for their word-finding difficulties.</p> <p>In <a href="https://direct.mit.edu/jocn/article-abstract/35/1/111/113588/Neural-Correlates-of-Naturally-Occurring-Speech">healthy adults</a>, occasional failures to name a picture of a common object are linked with changes in activity in brain regions that control motor aspects of speech, suggesting a spontaneous problem with articulation rather than a loss of word knowledge.</p> <p>In anomia due to primary progressive aphasia, brain regions that process word meanings show a loss of nerve cells and connections or <em><a href="https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0148707">atrophy</a></em>.</p> <p>Although anomic aphasia is common after strokes to the left hemisphere of the brain, the associated word-finding difficulties do not appear to be distinguishable by <a href="https://www.sciencedirect.com/science/article/pii/S0010945215003299">specific areas</a>.</p> <p>There are <a href="https://www.tandfonline.com/doi/abs/10.1080/02687030244000563">treatments</a> available for anomic aphasia. These will often involve speech pathologists training the individual on naming tasks using different kinds of cues or prompts to help retrieve words. The cues can be various meaningful features of objects and ideas, or sound features of words, or a combination of both. <a href="https://www.sciencedirect.com/science/article/pii/S002199241730014X">Smart tablet</a> and phone apps also show promise when used to complement therapy with home-based practice.</p> <p>The type of cue used for treatment is determined by the nature of the person’s impairment. Successful treatment is associated with changes in activity in <a href="https://www.sciencedirect.com/science/article/pii/S0093934X14000054">brain regions</a> known to support speech production. What do people think about when they go to sleep?

Instead, your brain is busy making detailed plans for the next day, replaying embarrassing <a href="https://www.tiktok.com/@ryanhdlombard/video/7052464974324583681?q=sleep%20thoughts&amp;t=1693536926124">moments</a> (“why did I say that?”), or producing seemingly random thoughts (“where is my birth certificate?”).</p> <p>Many social media users have shared <a href="https://www.tiktok.com/@komasawn/video/7267320333613419818">videos</a> on how to fall asleep faster by <a href="https://www.tiktok.com/@lilslvrtt/video/7225272823562997000">conjuring</a> up “<a href="https://www.tiktok.com/@ekai.is.okay/video/7169530076143439131?q=fake%20scenario%20fall%20asleep&amp;t=1693537172625">fake scenarios</a>”, such as a romance storyline where you’re the main character.</p> <p>But what does the research say? Does what we think about before bed influence how we sleep?</p> <h2>How you think in bed affects how you sleep</h2> <p>It turns out people who sleep well and those who sleep poorly have different kinds of thoughts before bed.</p> <p><a href="https://www.sciencedirect.com/science/article/pii/S1087079219302217">Good sleepers report</a> experiencing mostly visual sensory images as they drift to sleep – seeing people and objects, and having dream-like experiences.</p> <p>They may have less ordered thoughts and more hallucinatory experiences, such as imagining you’re participating in events in the real world.</p> <p>For people with insomnia, pre-sleep thoughts tend to be less visual and more focused on planning and problem-solving. These thoughts are also generally more unpleasant and less random than those of good sleepers.</p> <p>People with insomnia are also more likely to stress about sleep as they’re <em>trying</em> to sleep, leading to a vicious cycle; putting effort into sleep actually wakes you up more.</p> <p>People with insomnia often <a href="https://bpspsychub.onlinelibrary.wiley.com/doi/abs/10.1348/014466500163284">report</a> worrying, planning, or thinking about important things at bedtime, or focusing on problems or noises in the environment and having a general preoccupation with not sleeping.</p> <p>Unfortunately, all this pre-sleep mental activity can prevent you drifting off.</p> <p>One <a href="https://academic.oup.com/sleep/article/27/1/69/2707948">study</a> found even people who are normally good sleepers can have sleep problems if they’re stressed about something at bedtime (such as the prospect of having to give a speech when they wake up). Even <a href="https://pubmed.ncbi.nlm.nih.gov/17884278/">moderate levels of stress at bedtime</a> could affect sleep that night.</p> <p>Another <a href="https://jcsm.aasm.org/doi/full/10.5664/jcsm.6704">study</a> of 400 young adults looked at how binge viewing might affect sleep. The researchers found higher levels of binge viewing were associated with poorer sleep quality, more fatigue, and increased insomnia symptoms. “Cognitive arousal”, or mental activation, caused by an interesting narrative and identifying with characters, could play a role.</p> <p>The good news is there are techniques you can use to change the style and content of your pre-sleep thoughts. They could help reduce nighttime cognitive arousal or to replace unwanted thoughts with more pleasant ones. These techniques are called “cognitive refocusing”.</p> <h2>What is cognitive refocusing?</h2> <p>Cognitive refocusing, developed by US psychology researcher <a href="https://artsandsciences.syracuse.edu/people/faculty/gellis-phd-les-a/">Les Gellis</a>, involves distracting yourself with pleasant thoughts before bed. It’s like the “fake scenarios” social media users post about – but the trick is to think of a scenario that’s not <em>too</em> interesting.</p> <p>Decide <em>before</em> you go to bed what you’ll focus on as you lie there waiting for sleep to come.</p> <p>Pick an engaging cognitive task with enough scope and breadth to maintain your interest and attention – without causing emotional or physical arousal. So, nothing too scary, thrilling or stressful.</p> <p>For example, if you like interior decorating, you might imagine redesigning a room in your house.</p> <p>If you’re a football fan, you might mentally replay a passage of play or imagine a game plan.</p> <p>A music fan might mentally recite lyrics from their favourite album. A knitter might imagine knitting a blanket.</p> <p>Whatever you choose, make sure it’s suited to you and your interests. The task needs to feel pleasant, without being overstimulating.</p> <p>Cognitive refocusing is not a silver bullet, but it can help.</p> <p>One <a href="https://doi.org/10.1080/07448481.2022.2109031">study</a> of people with insomnia found those who tried cognitive refocusing had significant improvements in insomnia symptoms compared to a control group.</p> <h2>How ancient wisdom can help us sleep</h2> <p>Another age-old technique is mindfulness meditation.</p> <p>Meditation practice can increase our self-awareness and make us more aware of our thoughts. This can be useful for helping with rumination; often when we try to block or stop thoughts, it can make matters worse.</p> <p>Mindfulness training can help us recognise when we’re getting into a rumination spiral and allow us to sit back, almost like a passive observer.</p> <p>Try just watching the thoughts, without judgement. You might even like to say “hello” to your thoughts and just let them come and go. Allow them to be there and see them for what they are: just thoughts, nothing more.</p> <p><a href="https://doi.org/10.1007/s12671-021-01714-5">Research from our group</a> has shown mindfulness-based therapies can help people with insomnia. It may also help people with <a href="https://doi.org/10.1007/s11920-022-01370-z">psychiatric conditions</a> such as bipolar disorder, obsessive-compulsive disorder and schizophrenia get more sleep.</p> <h2>What can help ease your pre-sleep thoughts?</h2> <p>Good sleep starts the moment you wake up. To give yourself your best shot at a good night’s sleep, start by getting up at the same time each day and getting some morning light exposure (regardless of how much sleep you had the night before).</p> <p>Have a consistent bedtime, reduce technology use in the evening, and do regular exercise during the day.</p> <p>If your mind is busy at bedtime, try cognitive refocusing. Pick a “fake scenario” that will hold your attention but not be too scary or exciting. What not to say when someone comes out as LGBTQ+ (and what to say instead)

<p><strong>"Are you sure?"</strong></p> <p>People who identify as LGBTQ+ often struggle a great deal with their own feelings before they make the nerve-wracking decision to come out. Despite your best intentions, responding to this by asking, “Are you sure?” may be seen as calling into question the emotional journey that brought them to that point. </p> <p>Instead of expressing what might be interpreted as doubt, you could perhaps say, “It’s always good to know what you like and want. Thanks for coming out to me.”</p> <p><strong>"It must have been tough for your parents"</strong></p> <p>Yes, it could have been tough for the parents. But guess what? It was harder on the individual for even longer – and in more intense ways than you can imagine. Staying in the closet is not a very comfortable position for a self-accepting person, but it’s an even harder journey to get to that point of self-acceptance. LGBTQ+ individuals have battled real fears of being attacked, abandoned, discriminated against and disadvantaged. </p> <p>In fact, LGBTQ+ youth are three times more likely to commit suicide than their heterosexual counterparts. Although it’s reasonable to show empathy for their parents, it’s poorly-timed here. Instead, try, “I hope you are coping well with your parents’ response. I am here to talk if you need to. Thanks for coming out me.”</p> <p><strong>"Why didn't you tell me before?"</strong></p> <p>Well, it’s likely because they were afraid. It’s not because you are a monster, but because they love you and feared losing you as a result of coming out. The important thing is to not take it personally. When a loved one comes out, you are being called on to show targeted and urgent empathy. While your pain of being “left out” is real, it can wait a day or two. </p> <p>Right now, your focus should be how to celebrate your loved one’s life and the choices they’ve made, including the one of having just come out to you. Kick off the celebration by saying, “I am so proud that you have chosen to live your truth in front of me. Thanks for being authentic with me.”</p> <p><strong>"Are you the man or the woman in the relationship?"</strong></p> <p>More than anything, this question demonstrates a fundamental ignorance of the concept of same-sex love. It also misplaces the focus on the sexual act (rather than on identity) at a time when they’re in a vulnerable state. When a person comes out, they’re opening their hearts to you and sharing the emotional reality of who they are as individuals. </p> <p>Instead of asking this question, reinforce how grateful you are that the person has had the courage to come out to you, and say, “I accept you for who you are. Thank you for coming out to me.”</p> <p><strong>"Would you be my gay bestie?"</strong></p> <p>People who identify as LGBTQ+ can be good (or insufferable) company just like any other human being, but the point is, they are human beings and not objects or accessories as the pop culture depiction of the “gay bestie” might have you believe. </p> <p>Instead of this response, take this opportunity to reaffirm your loving, supportive relationship: “You are brave and honest, and this makes me respect you even more. Thanks for coming out to me.”</p> <p><strong>"I knew it!"</strong></p> <p>This particularly hurtful response is generally remarked when a “feminine” man or “butch” woman comes out. Many gender non-conforming individuals try very hard to either tone-down or manage their sexuality in public for fear of homophobia. </p> <p>By saying “I knew it!,” you’re inadvertently suggesting that their sexuality or gender non-conformity was being gossiped about behind their back in a sneaky, judgmental manner, which could, in turn, lead the person to feel that staying in the closet was a safer option than coming out. To avoid this situation, stick with a response of, “I’m honoured you chose to come out to me. Thank you.”</p> <p><strong>"But you're so masculine!"</strong></p> <p>On the opposite end of the spectrum lies this response, indicating disbelief that a “manly” man could be gay, or that a “feminine” woman could be lesbian. Being in the closet is an alienating experience; responding to an individual’s coming-out with a comment that suggests they don’t conform to their “new” sexual identity either can be just as alienating. </p> <p>A better response could be, simply, “I didn’t know, but I’m so grateful you came out to me. Why do I crave sugar and carbs when I’m sick?

You’re settling in on the couch for a sick day. Then you reach for the snacks.</p> <p>When you’re sick, your appetite often decreases. So why, at other times, do you crave sugary treats and carbohydrate-loaded comfort foods?</p> <p>A food <a href="https://pubmed.ncbi.nlm.nih.gov/28375878/">craving</a> goes beyond a mere desire to eat, it encompasses a <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7399671/#CR1">complex mix</a> of emotional, behavioural, cognitive and physiological processes. Whether it’s the need for a quick energy source or a temporary relief from discomfort, our bodies and minds work in tandem to drive our food preferences.</p> <p>Here we’ll explore the science behind why our bodies crave sugar and carbs – especially when we’re sick.</p> <h2>Fuelling the immune system</h2> <p>When sickness strikes, our immune system springs into action, requiring additional energy to combat invaders.</p> <p>This heightened activity often leads to an increase in our <a href="https://pubmed.ncbi.nlm.nih.gov/36505552/">metabolic rate</a>, energy demands and nutritional requirements.</p> <p>Sugary treats and carbs are quick sources of energy, satisfying this increased demand.</p> <p>But while a high sugar diet during times of illness may help meet increased metabolic demands, it could also exacerbate the immune and inflammatory response, potentially impeding recovery.</p> <p>In the longer term, high-sugar diets promote chronic <a href="https://pubmed.ncbi.nlm.nih.gov/33339337/">inflammation</a>, <a href="https://www.science.org/doi/10.1126/scitranslmed.aay6218?url_ver=Z39.88-2003&amp;rfr_id=ori:rid:crossref.org&amp;rfr_dat=cr_pub%20%200pubmed">alter gut microbiota</a> composition, and are associated with chronic disease. For a <a href="https://www.mdpi.com/2072-6643/12/4/1181">well-functioning immune system</a>, aim for a <a href="https://www.who.int/news-room/fact-sheets/detail/healthy-diet">balanced intake</a> of <a href="https://pubmed.ncbi.nlm.nih.gov/31267783/">fruits, vegetables</a>, fibre, protein, and low-glycaemic carbohydrates.</p> <h2>The stress response</h2> <p>Being sick is stressful for the body. Acute mild or intense stress, like we’d see if we’re sick, boosts the “<a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5921333/">flight or fight</a>” hormones adrenaline and cortisol. This mobilises stored energy to meet increased demands, but it can also curb appetite.</p> <p><a href="https://pubmed.ncbi.nlm.nih.gov/31125634/">Prolonged stress</a> can disrupt energy balance, and cause nutritional deficiencies and alterations in gut and brain functions. This can reduce a person’s threshold for craving sugar and salt, increasing their preferences towards energy-dense foods.</p> <p>The stress hormone cortisol can also increase your <a href="https://pubmed.ncbi.nlm.nih.gov/24123563/">preference</a> for high-calorie, comfort foods, which can <a href="https://pubmed.ncbi.nlm.nih.gov/36615866/">temporarily alleviate stress</a>.</p> <h2>The brain’s reward system</h2> <p>Comfort foods trigger your brain’s reward system, releasing feel-good neurotransmitters like <a href="https://pubmed.ncbi.nlm.nih.gov/30595479/">dopamine</a> and serotonin.</p> <p>But “<a href="https://pubmed.ncbi.nlm.nih.gov/30951762/">sugar rushes</a>” are often short-lived and can lead to decreased alertness and heightened fatigue within an hour of consumption.</p> <p>The link between carbohydrates (which the body converts to sugar) and serotonin can be traced back to 1971 when <a href="https://www.science.org/doi/10.1126/science.174.4013.1023?url_ver=Z39.88-2003&amp;rfr_id=ori:rid:crossref.org&amp;rfr_dat=cr_pub%20%200pubmed">researchers</a> found elevated tryptophan levels (serotonin’s precursor) in rats’ plasma and brains after a carbohydrate-rich diet.</p> <p>Subsequent studies in humans established connections between carbohydrates and mood, especially in relation to <a href="https://pubmed.ncbi.nlm.nih.gov/2903717/">obesity, depression and seasonal affective disorder</a>. Therapies enhancing serotonin have since been shown to <a href="https://pubmed.ncbi.nlm.nih.gov/2903717/">reduce carbohydrate intake</a>.</p> <p>Remarkably, around <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8911970/pdf/molecules-27-01680.pdf">90% of serotonin</a> production occurs in the gut. The vast microbial population in our gut exerts a potent influence on <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8106557/">immunity, metabolism</a> and <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8293578/pdf/40168_2021_Article_1093.pdf">appetite</a>.</p> <p>Recent mouse studies have even identified specific microbes linked to <a href="https://www.cell.com/current-biology/fulltext/S0960-9822(22)01750-X">sugar binges after antibiotic treatment</a>.</p> <h2>Some people eat less when they’re sick</h2> <p>Not everyone craves sugar and carbs when they are sick. Some people eat less for a few reasons:</p> <ul> <li> <p>they have less of an appetite. While <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5610818/pdf/JDR2017-4527980.pdf">ghrelin</a> (the “hunger” hormone) levels might initially rise, prolonged illness can suppress appetite due to nausea, fatigue and discomfort. <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5921333/">Critically ill</a> patients have reduced food intake and are at risk of malnutrition</p> </li> <li> <p><a href="https://pubmed.ncbi.nlm.nih.gov/30777142/">metabolic adaptation</a>. The body might slow specific metabolic processes to conserve energy, reducing overall calorie requirements</p> </li> <li> <p>altered taste perception. <a href="https://pubmed.ncbi.nlm.nih.gov/32195512/#:%7E:text=The%20ability%20of%20an%20individual%20to%20perceive%20tastes,intake%2C%20playing%20an%20important%20role%20in%20promoting%20satiation%2Fsatiety.">Taste</a> is an important component that affects both appetite and energy intake. Alterations in taste and smell is a common symptom when we are sick and was common with <a href="https://doi.org/10.1101/2020.04.05.20048421">COVID</a></p> </li> <li> <p>consuming fluids like water, tea or broths might be more appealing and manageable than solid foods. Why we’re more prone to car-sickness when we set off on holiday

Nearly <a href="https://www.autonomicneuroscience.com/article/S1566-0702(06)00212-8/fulltext">a third of people</a> experience motion sickness – and to this day we don’t exactly know what causes it. The prevailing theory suggests it is triggered by a <a href="https://onlinelibrary.wiley.com/doi/10.1111/cns.12468">poor perception of movement</a>.</p> <p>Departure to and return from summer holidays seem moments especially prone to this sickness’ stealthy advances. We (or at least those of us inclined to travel sickness) are more often ill during these particular journeys than during our normal comings and goings.</p> <p>Let’s note too that lots of travellers feel a sense of fatigue, drowsiness, apathy or lack of energy without having done any particularly exhausting activity. These are in fact symptoms of <a href="https://www.sciencedirect.com/science/article/abs/pii/S0094576598001532">mild motion sickness</a>, which show that many more people are affected by the condition than you’d think.</p> <p>Why this apparent heightened susceptibility during holiday trips? There are many reasons. Compared to normal travel, these journeys feature certain conditions, all with the potential to increase the incidence and severity of symptoms. Here are some pieces of explanation, and advice to minimise the risk.</p> <h2>Long journeys – repetition of movements that make you queasy</h2> <p>In a car, the further one travels, the more likely one is to feel ill, as shown by a number of <a href="https://journals.sagepub.com/doi/10.1243/0954407042580093">mathematical models which predict motion sickness</a>.</p> <p>It’s the adding up of unpleasant movements which takes us over the threshold where we feel symptoms. For certain people, this <a href="https://www.sciencedirect.com/science/article/abs/pii/S1566070206002128">can happen after just a few minutes</a>); for others it develops more slowly. Only on long journeys, after several hours on the road, in the air or on a boat, will this latter group be pushed over their limit and start feeling unwell.</p> <p>Activities undertaken to pass the time during a long journey could add to feelings of queasiness. Often people do something to occupy and entertain themselves: read a book, watch a film, play a video game or scroll through social media. Except, these visually stimulating activities absorb our attention to the point that we’re not tuned in to the visual cues that allows our brain to assess the movement of the vehicle. This creates a confusion in the perception of movement. As a result, it becomes <a href="https://www.sciencedirect.com/science/article/abs/pii/S0141938214000043?via%3Dihub">much easier to feel sick</a>.</p> <h2>Journey conditions: risks adding up</h2> <p>In summer, the temperature inside a vehicle is difficult to control, with the sun often imposing a stifling heat; conditions which <a href="https://www.ingentaconnect.com/content/asma/asem/2013/00000084/00000005/art00004">tend to accentuate the symptoms of motion sickness</a>.</p> <p>When it’s hot, <a href="https://theconversation.com/lesquels-de-nos-organes-sont-les-plus-menaces-par-la-canicule-119563">our body has to make an effort to regulate its temperature</a>, through sweat or breathing for example. These various signals amount to ‘primary symptoms’ as they can contribute to the appearance of other more substantive symptoms: dilation of the blood vessels, sickness, nausea or vomiting, as applicable.</p> <p>To counter these effects, one is tempted to switch the air conditioning on, which could itself, perversely, <a href="https://www.francetvinfo.fr/sante/environnement-et-sante/la-climatisation-rend-elle-malade_2885673.html">worsen the situation for passengers highly susceptible to motion sickness</a>. Ventilation and cabin air systems also push people toward their nausea thresholds.</p> <p>Unpleasant smells are another factor that can <a href="https://www.ingentaconnect.com/content/asma/asem/2013/00000084/00000005/art00004">accentuate car sickness symptoms</a>: traffic fumes, cigarette smoke, fetid air or even <a href="https://link.springer.com/article/10.1007/s00221-015-4209-9">the smell of leather</a> were identified as <a href="https://www.sciencedirect.com/science/article/pii/S1369847819306539">second most common cause of car sickness</a>! These are bigger risk factors at the start of holiday season, when <a href="https://www.francetvinfo.fr/economie/transports/trafic/vacances-les-vagues-de-departs-massifs-sont-associees-a-de-fortes-emissions-de-polluants_2839361.html">air pollution peaks regularly</a> and the sun’s rays heat up materials. It’s also known that there is a region of the brain – the area postrema or chemoreceptor trigger zone – which can trigger over-production of saliva and nausea specifically when certain smells are detected – a protective reflex against toxins and other poisonous substances.</p> <h2>Traffic: a physical and mental imposition</h2> <p>In a car, it isn’t speed that makes one ill but <a href="https://www.tandfonline.com/doi/abs/10.1080/001401399184730">changes in speed</a>, especially abrupt ones. Acceleration and breaking movements aggravate the human body, even more than turning corners.</p> <p>In practice, variations in speed are often forced on the driver by road design (speed limits, crossings, traffic lights), but also by the state of the traffic. A car stuck in jams will be forced to speed up and slow down at random intervals, <a href="https://www.researchgate.net/publication/366836220_Effect_of_Horizontal_Acceleration_and_Seat_Orientation_on_Motion_Sickness_in_Passenger_Cars">which grates, even at low speeds</a>.</p> <p>Traffic jams also have a psychological element. Delays to a journey (which might already have been very long), anxiousness about arriving at the arranged time, which is looking less and less likely, tiredness, stress and irritation can all cause the passengers’ mood to crash. It’s been observed that these factors <a href="https://journals.sagepub.com/doi/10.1177/0018720819876139">significantly impact the degree of motion sickness symptoms</a>. It would be better to take these setbacks calmly and stay in a relaxed frame of mind but that’s of course easier said than done.</p> <h2>Some tips to limit the damage</h2> <p>If you’re driving with passengers with a tendency to be car sick, or you’re susceptible yourself, some adjustments to your travel habits might help you.</p> <p><strong>For the driver:</strong></p> <ul> <li> <p><em>Take regular breaks</em>. This allows passengers to take a breather, and to reduce to a significant extent or even get over their symptoms. Sometimes symptoms can take a while to disappear but <a href="https://content.iospress.com/articles/journal-of-vestibular-research/ves7-6-01">generally 15-30 minutes is enough</a>.</p> </li> <li> <p><em>Try to cut down the amount of hard acceleration and braking you do</em>. Keep as far as possible to the same speed and adopt a smooth driving style, including when you overtake or brake.</p> </li> <li> <p><em>Avoid taking corners too sharply on winding roads.</em> Passengers should be <a href="https://www.tandfonline.com/doi/abs/10.1080/00140139.2015.1109713">jolted in their seats as little as possible</a>.</p> </li> </ul> <p><strong>For passengers</strong></p> <ul> <li> <p><a href="https://www.tandfonline.com/doi/abs/10.1080/001401399184730"><em>Sit as far forward in the vehicle as possible</em></a>. Any movement while travelling is better absorbed by the body from this position. <a href="https://www.tandfonline.com/doi/abs/10.1080/00140139108964831">It’s in the driver’s seat that people are least affected by car sickness</a>, since one has control over the vehicle’s movement.</p> </li> <li> <p><em>Avoid looking at screens and other visual content (books, etc.)</em>, particularly when the vehicle isn’t moving at a constant speed. Instead, <a href="https://bpspsychub.onlinelibrary.wiley.com/doi/abs/10.1348/000712699161594">try and look forward out of the window</a>, towards the horizon.</p> </li> <li> <p><a href="https://www.tandfonline.com/doi/abs/10.1080/00140139.2015.1109713"><em>Shut your eyes</em></a> or <a href="https://psycnet.apa.org/record/1976-12574-000"><em>try to sleep</em></a>. Slowing down activity soothes the body.</p> </li> <li> <p><a href="https://link.springer.com/chapter/10.1007/978-3-030-27928-8_26"><em>Tilt your seat back</em></a>. This allows you to be less destabilised by the vehicle’s movements</p> </li> <li> <p><em>Go for car games</em> with the other passengers if you get bored: play “I Spy”, <a href="https://theses.gla.ac.uk/80069/1/13905209.pdf">sing songs</a>, count cars of a particular colour or make, and other old favourites of proven effectiveness to help pass the time and, above all, <a href="http://iospress.com/articles/journal-of-vestibular-research/ves00541">take your attention away from the queasiness</a>. The emergence and disappearance of symptoms is mainly a psychological phenomenon.</p> </li> </ul> <p>Finally, given the role of the mind in car sickness symptoms, note that passengers experiencing queasiness can feel better with a placebo (something with no proven medicinal value but presented to them as a magic cure). Simple tips <a href="https://academic.oup.com/jtm/article/5/2/89/1801039">have been shown to be particularly effective</a>. Surprise! Scientists find falls likely when texting and walking

<p>It seems obvious that texting while walking is <a href="https://cosmosmagazine.com/people/behaviour/millennials-most-likely-to-text-and-drive/" target="_blank" rel="noreferrer noopener">risky</a> business. But while there has been plenty of research showing it’s a dangerous distraction, <a href="https://www.sciencedirect.com/science/article/abs/pii/S0966636217309670" target="_blank" rel="noreferrer noopener">some</a> <a href="https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0179802" target="_blank" rel="noreferrer noopener">studies</a> have suggested that younger people are better at negotiating obstacles while on their phones.</p> <div class="copy"> <p>A study <a href="https://dx.doi.org/10.1016/j.heliyon.2023.e18366" target="_blank" rel="noreferrer noopener">published</a> in <em>Heliyon</em> has refuted this, finding that university students are more likely to fall if they walk while texting.</p> <p>It also found they’re less accurate texters while walking.</p> <p>“On any day it seems as many as 80% of people, both younger and older, may be head down and texting. I wondered: is this safe?” said senior author Dr Matthew Brodie, a neuroscientist and engineer at the University of New South Wales.</p> <p>“This has made me want to investigate the dangers of texting while walking. I wanted to know if these dangers are real or imagined and to measure the risk in a repeatable way.”</p> <p>Brodie and colleagues recruited 50 undergraduate students from UNSW to take part in the study.</p> <p>Participants walked across a specially built tiled surface, fitted with a tile that could slip out halfway through.</p> <p>They were asked to either walk across the surface normally, or walk across it while texting “the quick brown fox jumps over the lazy dog”.</p> <p>The students were strapped to safety harnesses so they couldn’t fall, and told they may or may not slip.</p> <div style="position: relative; display: block; max-width: 100%;"> <div style="padding-top: 56.25%;"><iframe style="position: absolute; top: 0px; right: 0px; bottom: 0px; left: 0px; width: 100%; height: 100%;" src="https://players.brightcove.net/5483960636001/HJH3i8Guf_default/index.html?videoId=6332776122112" allowfullscreen="allowfullscreen"></iframe></div> </div> <p class="caption"><em>The methods used in the experiment. Credit: Heliyon Brodie et al.</em></p> <p>“What surprised me is how differently people responded to the threat of slipping,” says Brodie.</p> <p>“Some slowed down and took a more cautious approach. Others sped up in anticipation of slipping. Such different approaches reinforce how no two people are the same, and to better prevent accidents from texting while walking, multiple strategies may be needed.”</p> <p>The researchers recorded motion data from the students as they moved and slipped, analysing how stable they were.</p> <p>They found that texting while walking made the students significantly less stable.</p> <p>Specifically, the students “trunk angle” – the angle of their torsos – varied more when they were slipping while texting. Why do I fall asleep on the sofa but am wide awake when I get to bed?

The room is nice and warm, the sofa is comfortable, and the background noise of the TV lulls you to sleep.</p> <p>Then a loved one nudges you awake and reminds you to go sleep – in bed. But when you get there, you find to your frustration that you’re wide awake.</p> <p>Why does sleep come so easily on the sofa but not always in bed?</p> <h2>Why is it so easy to fall asleep on the sofa?</h2> <p><a href="https://www.sleepfoundation.org/circadian-rhythm/sleep-drive-and-your-body-clock#:%7E:text=Sleep%2Fwake%20homeostasis%20balances%20our,that%20it's%20time%20to%20sleep.">Sleep pressure</a> is one reason why you fall asleep on the sofa. This refers to the strength of the biological drive for sleep. The longer you’ve been awake, the greater the sleep pressure.</p> <p>Your body clock or <a href="https://www.thensf.org/what-is-a-circadian-rhythm/">circadian rhythm</a> is another factor. This tells you to be awake during the day and to sleep at night.</p> <p>Your environment will also impact how likely it is you fall asleep. You might have just <a href="https://theconversation.com/health-check-food-comas-or-why-eating-sometimes-makes-you-sleepy-44355">eaten a meal</a>, your very comfortable sofa is in a warm room, with dim lighting and maybe a TV program in the background. For many people, this environment is perfect for falling asleep.</p> <p>So by the end of the day, sleep pressure is strong, your circadian rhythm is telling you it’s time for sleep <em>and</em> your environment is cosy and comfortable.</p> <h2>What happens after a nap on the sofa?</h2> <p>If you’ve had a nap on the sofa before heading to bed, your sleep pressure is likely much lower than it was before your nap. Instead of having more than 16 hours of wakefulness behind you, you’ve just woken up and therefore have less sleep pressure. This can make it much harder to fall asleep in bed.</p> <p>If you just fell asleep on the sofa for five minutes, you might not have too much trouble getting to sleep in bed. This is because a nap that short is unlikely to reduce your sleep pressure very much. But if you were asleep for an hour, it might be a different story.</p> <p>Your sleep cycles might also be working against you. Most sleep cycles are about <a href="https://www.researchgate.net/profile/Mary-Carskadon/publication/287231408_Normal_Human_Sleep_An_Overview_Principles_and_Practice_of_Sleep_Medicine_MH_Kryger_Ed/links/5db0c338299bf111d4c026c5/Normal-Human-Sleep-An-Overview-Principles-and-Practice-of-Sleep-Medicine-MH-Kryger-Ed.pdf">90 minutes long</a>. They start with light sleep, progress to deep sleep, and then end with light sleep again. If you wake up during deep sleep, you’re probably going to feel groggy – and it might be easy to get back to sleep when you go to bed. But if you wake up during light sleep it could be harder to fall asleep again in bed.</p> <p>The activities you might do when you get up from the sofa – like turning on bright lights or brushing your teeth – can also make you feel more alert and make it harder to sleep when you get to bed.</p> <h2>Why can’t I fall asleep in my own bed?</h2> <p>There are other reasons why falling also in your bed could be challenging. Many people experience anxiety about falling asleep. They <a href="https://pubmed.ncbi.nlm.nih.gov/24005330/">worry</a> about getting enough sleep or falling asleep fast enough.</p> <p>In such cases, getting into bed can be associated with feelings of stress and apprehension, which make it even harder to sleep. It might be easier to fall asleep on the couch, where there is less stress involved.</p> <p>It might also be harder to fall asleep in bed because of poor <a href="https://www.sleepfoundation.org/sleep-hygiene">sleep hygiene</a>. This refers to your pre-sleep behaviours and sleep environment.</p> <p>Good sleep hygiene, or healthy sleep habits, includes having a regular routine before bed, a dark, quiet room to sleep in, and not using your mobile phone in bed. For many people who don’t have good sleep hygiene, their behaviours before bed and their bedroom environment might not be conducive to sleep.</p> <h2>How can I make it easier to fall asleep in bed?</h2> <p>First, make sure your room is dark, quiet and comfortable. In winter this might mean putting a heater on 20 minutes before you go to bed or taking a heat pack to bed with you. In summer, you might consider air conditioning or a fan to make your bedroom comfortable for sleeping.</p> <p>If you find it easy to fall asleep with the TV on, you might like to play “<a href="https://www.youtube.com/watch?v=2iL1Ce1PZFM&amp;t=1s">white noise</a>” in your bedroom as you fall asleep. Some evidence suggests this may make it <a href="https://www.sciencedirect.com/science/article/pii/S1087079220301283#abs0010">easier to fall asleep</a> by masking other disruptive noises.</p> <p>Your behaviour before bed also impacts how easy it is to fall asleep. Making sure you follow the same bedtime routine every night (including going to bed at the same time) can help.</p> <p>Also, even though it’s hard, try not to look at your phone while you’re in bed. What happens in our body when we encounter and fight off a virus like the flu, SARS-CoV-2 or RSV?

Our immune system uses a number of strategies to fight off viral infections. Let’s look at how it does this.</p> <h2>First line of defence</h2> <p>When we encounter respiratory viruses, the <a href="https://www.sciencedirect.com/science/article/pii/S193131281600038X?via%3Dihub/">first line of defence</a> is the physical and chemical barriers in our nose, upper airways, and lungs. Barriers like the mucus lining and hair-like structures on the surface of cells, work together to trap and remove viruses before they can reach deeper into our respiratory system.</p> <p>Our defence also includes our behaviours such as coughing or sneezing. When we blow our nose, the mucus, viruses, and any other pathogens that are caught within it are expelled.</p> <p>But sometimes, viruses manage to evade these initial barriers and sneak into our respiratory system. This activates the cells of our innate immune system.</p> <h2>Patrolling for potential invaders</h2> <p>While our acquired immune system develops over time, our innate immune system is present at birth. It generates “non-specific” immunity by identifying what’s foreign. The cells of innate immunity act like a patrol system, searching for any invaders. These innate cells patrol almost every part of our body, from our skin to our nose, lungs and even internal organs.</p> <p>Our respiratory system has different type of innate cells such – as macrophages, neutrophils and natural killer cells – which patrol in our body looking for intruders. If they recognise anything foreign, in this case a virus, they will initiate an attack response.</p> <p>Each cell type plays a slightly different role. Macrophages, for example, will not only engulf and digest viruses (phagocytosis) but also release a cocktail of different molecules (cytokines) that will warn and recruit other cells to <a href="https://onlinelibrary.wiley.com/doi/full/10.1111/cmi.12580">fight against the danger</a>.</p> <p>In the meantime, natural killer cells, aptly named, attack infected cells, and stop viruses from multiplying and <a href="https://www.nature.com/articles/s41577-021-00558-3">invading our body further</a>.</p> <p>Natural killer cells also promote inflammation, a <a href="https://www.hindawi.com/journals/jir/2018/1467538/">crucial part of the immune response</a>. It helps to recruit more immune cells to the site of infection, enhances blood flow, and increases the permeability of blood vessels, allowing immune cells to reach the infected tissues. At this stage, our immune system is fighting a war against viruses and the result can cause inflammation, fevers, coughs and congestion.</p> <h2>Launching a specific attack</h2> <p>As the innate immune response begins, another branch of the immune system called the adaptive immune system is <a href="https://www.ncbi.nlm.nih.gov/books/NBK21070/">activated</a>.</p> <p>The adaptive immune system is more specific than the innate immune system, and it decides on the correct tools and strategy to fight off the viral invaders. This system plays a vital role in eliminating the virus and providing long-term protection against future infections.</p> <p>Specialised cells called T cells and B cells are key players in acquired immunity.</p> <p>T cells (specifically, helper T cells and cytotoxic T cells) recognise viral proteins on the surface of infected cells:</p> <ul> <li> <p>helper T cells release molecules that <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3764486/">further activate immune cells</a></p> </li> <li> <p>cytotoxic T cells directly kill infected cells with a very great precision, <a href="https://www.frontiersin.org/articles/10.3389/fimmu.2018.00678/full">avoiding any healthy cells around</a>.</p> </li> </ul> <p>B cells produce antibodies, which are proteins that can bind to viruses, neutralise them, and mark them for <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7247032/">destruction by other immune cells</a>.</p> <p>B cells are a critical part of memory in our immune system. They will remember what happened and won’t forget for years. When the same virus attacks again, B cells will be ready to fight it off and will neutralise it faster and better.</p> <p>Thanks to the adaptive immune system, vaccines for respiratory viruses such as the COVID mRNA vaccine keep us protected from <a href="https://www.health.gov.au/our-work/covid-19-vaccines/our-vaccines/how-they-work">being sick or severely ill</a>. However, if the same virus became mutated, our immune system will act as if it was a new virus and will have to fight in a war again.</p> <h2>Neutralising the threat</h2> <p>As the immune response progresses, the combined efforts of the innate and adaptive immune systems helps control the virus. Infected cells are cleared, and the virus is neutralised and eliminated from the body.</p> <p>As the infection subsides, symptoms gradually improve, and we begin to feel better and to recover.</p> <p>But recovery varies depending on the specific virus and us as individuals. Some respiratory viruses, like rhinoviruses which cause the common cold, may cause relatively mild symptoms and a quick recovery. Weight loss: why you don’t just lose fat when you’re on a diet

<p><em><a href="https://theconversation.com/profiles/adam-collins-1179004">Adam Collins</a>, <a href="https://theconversation.com/institutions/university-of-surrey-1201">University of Surrey</a></em></p> <p>When you go on a diet, you don’t just lose fat – you lose muscle too. This can have many repercussions – not only on your fitness and strength, but on your metabolism.</p> <p>To lose weight (body fat), you need to be in a calorie deficit. This means consuming fewer calories than your body uses, or exercising to burn more calories than you consume.</p> <p>During the first few days in a calorie deficit, the body uses up its small reservoir of <a href="https://pubmed.ncbi.nlm.nih.gov/1615908/">glycogen stores</a> for energy. Glycogen is a string of glucose (sugar) that comes from the carbohydrates you eat. Since carbs are the body’s main energy source, this is why any glucose the body doesn’t immediately use is stored to use for energy later.</p> <p>But as carbohydrate molecules bind with water, this means that when the body stores glycogen, it also stores water in the muscles. As these glycogen stores are used up, the body also releases a significant amount of water. This is often called “water weight”, and explains why some may feel they lose considerable weight early in their diet.</p> <p>Given you only have days’ worth of glycogen stores, this is why the body uses fat to store extra calories for when you need it. Once the glycogen stores are used up, the body shifts to metabolising fat to get the energy it needs to function.</p> <p>But not all tissues can use fat for energy – such as the brain. This is why the body needs to metabolise your muscles when you’re in a calorie deficit.</p> <p>Protein (from the food you eat) is stored in your muscles. The body can convert this stored protein into glucose for energy. But this means you subsequently lose the muscle tissue itself when that happens. This has significant consequences – including <a href="https://pubmed.ncbi.nlm.nih.gov/35103583/">slowing the metabolism</a>, which may ultimately drive weight regain after losing weight.</p> <h2>Muscle loss</h2> <p>Many factors can affect how much muscle you lose while in a calorie deficit.</p> <p>While it was once thought that the more fat you had, the <a href="https://pubmed.ncbi.nlm.nih.gov/10673906/">less muscle you lost</a> in a calorie deficit, this has since been disproved – with both <a href="https://pubmed.ncbi.nlm.nih.gov/3066619/">lean</a> and <a href="https://pubmed.ncbi.nlm.nih.gov/17367567/">obese people</a> losing significant rates of muscle when dieting.</p> <p><a href="https://pubmed.ncbi.nlm.nih.gov/29957829/">Ethnicity</a> and <a href="https://pubmed.ncbi.nlm.nih.gov/32734691/">genetics</a> may, however, play a role – with studies showing black people tend to lose more muscle mass in a calorie deficit than white people do. Some research also suggests that genetic variants may make some people more susceptible to <a href="https://pubmed.ncbi.nlm.nih.gov/32734691/">certain dietary changes</a>, which may determine how much muscle mass they end up losing.</p> <p>Muscle loss will also happen regardless of whether you lose weight <a href="https://pubmed.ncbi.nlm.nih.gov/30925026/">gradually or quickly</a>. A better determinant of how much muscle you’ll lose depends on <a href="https://pubmed.ncbi.nlm.nih.gov/30925026/">how much weight you end up losing</a>. If a person loses 10% of their body weight, typically around 20% of this is fat-free mass (the proportion of body mass that isn’t fat – such as muscle). This can equate to several kilograms of muscle.</p> <p>Many people also think that what you eat while losing weight may determine how much muscle you lose, with it commonly believed that if you eat plenty of protein you’re less likely to lose muscle mass. This is <a href="https://pubmed.ncbi.nlm.nih.gov/19246357/">debatable</a>, with research showing people lose as much muscle on high-protein weight loss diets as people who followed other types of diets.</p> <p>Low-carb diets have also been claimed to promote more fat loss. But studies comparing <a href="https://pubmed.ncbi.nlm.nih.gov/22258266/">different types of diets</a> have found that low-fat high-carb diets seem to offer <a href="https://pubmed.ncbi.nlm.nih.gov/28193517/">the same, if not better, fat loss</a> than low-carb, high-fat diets – with no differences in muscle loss.</p> <h2>Protein and exercise</h2> <p>Given all that has been said, the only way to prevent muscle loss somewhat while losing weight is to combine exercise (particularly <a href="https://pubmed.ncbi.nlm.nih.gov/18356845/">resistance exercise</a> and <a href="https://pubmed.ncbi.nlm.nih.gov/28507015/">endurance exercise</a>) with a diet higher in protein. This is because exercise stimulates muscle growth – but this process can only happen if you have an adequate supply of protein.</p> <p>It’s suggested adults normally aim to consume <a href="https://pubmed.ncbi.nlm.nih.gov/34371981/">0.8g of protein per kilogram of body weight</a> per day to maintain muscle mass. But given the extra demand exercise places on the muscles, a person will probably need to consume 1.2-1.5g of protein per kilogram of body weight to preserve muscle during weight loss. People who exercise a lot may need to increase that to more than <a href="https://pubmed.ncbi.nlm.nih.gov/28642676/">2g per kilogram of body weight</a> when losing weight. <a href="https://pubmed.ncbi.nlm.nih.gov/22221216/">Older people</a> may also need to consume more protein than average.</p> <p>Just be wary of consuming too much protein (more than 2.5g per kilogram of body weight) as eating more than your body uses could have an <a href="https://pubmed.ncbi.nlm.nih.gov/28507015/">adverse effect</a> on your metabolism by potentially making the body less able to draw upon glucose for energy. It may also put greater pressure on the kidneys and liver – which could lead to serious <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7460905/#:%7E:text=High%20dietary%20protein%20intake%20can,a%20role%20in%20kidney%20health.">health issues</a>, such as liver and kidney damage.</p> <p>Even if you prevent muscle loss when losing weight, other metabolic changes still happen that promote weight regain – such as changes in your metabolic rate (the minimum amount of calories your body needs to survive) and <a href="https://pubmed.ncbi.nlm.nih.gov/21677272/">increases in appetite and hunger</a>. This is why, when trying to lose weight, the most important thing to consider is how sustainable your diet and lifestyle changes are. Paracetamol versus ibuprofen – which works best and when?

So deciding which one you should choose is dependent on the type of pain you are experiencing. Sometimes it might be appropriate to take a medication that contains both drugs.</p> <p>In Australia, <a href="https://www.tga.gov.au/paracetamol-practitioner-fact-sheet#:%7E:text=It%20is%20available%20in%20many,Panamax%2C%20Chemist%20Own%20and%20Dymadon.">paracetamol</a> is branded as Panadol, Herron Paracetamol, Panamax, Chemist Own or Dymadon, plus there are generic chemist brands. Nurofen is the common brand name for <a href="https://www.ebs.tga.gov.au/ebs/picmi/picmirepository.nsf/PICMI?OpenForm&amp;t=&amp;q=ibuprofen">ibuprofen</a>, which is also sold under generic brand names.</p> <p>So how do you know which one to choose and when?</p> <h2>Different blocking actions</h2> <p>While ibuprofen and paracetamol can be taken for similar reasons (pain relief) each works in a slightly different way.</p> <p>Ibuprofen is a <a href="https://www.healthdirect.gov.au/anti-inflammatory-medicines">non-steroidal anti-inflammatory drug</a>, which means it acts by blocking the enzymes that produce a group of chemicals called <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3081099/">prostaglandins</a>. These chemicals are important for normal body functions such as relaxing blood vessels, preventing blood clotting, secreting protective mucus in the gut and helping the uterus contract. They are also involved in inflammation, pain and fever.</p> <p>It is still not completely understood how paracetamol works. Like ibuprofen, it is thought to act by blocking the enzymes that produce prostaglandins, although through a different mechanism to ibuprofen. There is also good evidence paracetamol <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7590544/">interacts</a> with the brain’s <a href="https://www.healthline.com/health/endocannabinoid-system">endocannabinoid system</a> and the “<a href="https://www.physio-pedia.com/Pain_Descending_Pathways">descending pain pathway</a>”, which inhibits the perception of pain.</p> <h2>Is one drug better than the other?</h2> <p>Because they each provide pain relief in different ways, paracetamol can be better at treating some types of pain, while ibuprofen is better at treating other types. But be wary of packaging that claims a medication is useful for targeting pain associated with a specific condition as these claims are <a href="https://www.abc.net.au/news/2017-08-03/nurofen-offers-3.5-million-compensation-to-customers/8770910">not true</a>.</p> <p>Because it reduces inflammation, the <a href="https://www.tg.org.au/">Australian Therapeutic Guidelines</a> state ibuprofen is the better choice for pain associated with osteo- and rheumatoid arthritis, period pain, some types of headache, and for pain that comes from having an operation. Paracetamol does not reduce inflammation but it is a better choice when fever is associated with the pain, like when you have a cold or flu.</p> <p>The Australian government recommends either paracetamol or ibuprofen if you have <a href="https://www.healthdirect.gov.au/covid-19/treating-symptoms-at-home">pain associated with COVID</a>.</p> <h2>What about taking both or ‘piggybacking’ them at intervals?</h2> <p>We can sometimes get better relief when we take both types of medicine at the same time, since each targets a different cause or pathway of the pain. If one pathway does not completely control the pain then it can be useful to target the other one. The effects of each drug <a href="https://www.frontiersin.org/articles/10.3389/fphar.2017.00158/full">can add</a> together for a bigger effect.</p> <p>Combination products that contain both paracetamol and ibuprofen in a single tablet include <a href="https://www.ebs.tga.gov.au/ebs/picmi/picmirepository.nsf/pdf?OpenAgent&amp;id=CP-2022-CMI-02442-1&amp;d=20230620172310101">Nuromol</a> and <a href="https://maxigesic.com.au/wp-content/uploads/2021/05/cmi-approved.pdf">Maxigesic</a>.</p> <p>Using a combination product means you can take fewer tablets. However, the doses in these combined products are sometimes less than the maximum recommended dose, meaning they might not work as well when compared with taking the tablets individually.</p> <p>Other times, you can get the best effect by alternating doses of ibuprofen and paracetamol. This keeps the levels of the medication in the body more constant and helps to provide more steady pain relief. This may be particularly useful when treating <a href="https://www.rch.org.au/kidsinfo/fact_sheets/Pain_relief_for_children_-_Paracetamol_and_Ibuprofen/#:%7E:text=So%20that%20your%20child%27s%20pain,too%20much%20of%20either%20medicine.">pain</a> and <a href="https://www.cochranelibrary.com/cdsr/doi/10.1002/14651858.CD009572.pub2/full?highlightAbstract=therapy%7Cfor%7Cibuprofen%7Calternating%7Cchildren%7Creview%7Cfour%7Ctherapi%7Caltern%7Ccombin%7Cfebril%7Ccombined%7Cparacetamol%7Cfebrile%7Cchild">fever</a> in children. To do this, one drug is given, then a dose of the other drug is given a few hours later, with you continuing to alternate between the two throughout the day.</p> <p>If you are alternating between different pain medicines, make sure you leave <a href="https://www.nhs.uk/medicines/paracetamol-for-children/taking-paracetamol-for-children-with-other-medicines-and-herbal-supplements/#:%7E:text=If%20you%27ve%20given%20your%20child%20paracetamol%20and%20they%27re,1%20medicine%20at%20a%20time">time (at least one hour)</a> between the dosing of each product to get more effective and consistent relief. Only give the recommended dose of each medicine as outlined on the pack. And do not administer more than the maximum recommended number of doses for each medicine per day.</p> <h2>How do the side effects compare?</h2> <p>Side effects from either drug are rare and generally mild.</p> <p><a href="https://www.tga.gov.au/sites/default/files/otc-template-pi-ibuprofen.rtf">Ibuprofen</a> does have a <a href="https://www.webmd.com/rheumatoid-arthritis/features/anti-inflammatory-drugs-rheumatoid-arthritis">reputation</a> for causing stomach problems. These can manifest as nausea, indigestion, bleeding in the stomach, and diarrhoea. For this reason, people with a history of bleeding or ulcers in the gut should not take ibuprofen. Ibuprofen is also known to sometimes cause headaches, dizziness, and higher blood pressure.</p> <p>Because ibuprofen thins the blood, it should also not be taken by people who are taking other medicines to thin the blood; like aspirin, warfarin, and clopidogrel. Ibuprofen should also be <a href="https://www.tga.gov.au/sites/default/files/otc-template-pi-ibuprofen.rtf">avoided</a> by pregnant women and people with asthma. In these cases, paracetamol is the better choice.</p> <p>However, you need to be careful when using these medicines to make sure you don’t use more than is recommended. This is particularly important for <a href="https://onlinelibrary.wiley.com/doi/full/10.5694/mja2.50296">paracetamol</a>.</p> <p>Paracetamol at the recommended doses is not toxic but too much can lead to liver failure.</p> <p>Because paracetamol is found in lots of different products, it can be hard to keep track of exactly how much paracetamol you have taken and this increases the risk of taking too much.</p> <h2>Both work, both need to be used safely</h2> <p>Paracetamol and ibuprofen are effective medications for the relief of both pain and fever; however, care must be taken to use them safely.</p> <p>Always read the label so you know exactly what products you are using and how much. Only take the recommended dose, and if you need to, write down the time you take each dose. Why does my back get so sore when I’m sick? The connection between immunity and pain

Or COVID?</p> <p>This discomfort, common during many illnesses, is not just a random symptom. It’s a result of complex interactions between your immune system and your brain called the “<a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3314453/">neuroimmune synapse</a>”.</p> <p>A fascinating and yet-to-be-understood consequence of this conversation between the immune and brain systems during sickness is that it is particularly noticeable in the <a href="https://academic.oup.com/brain/article/145/3/1098/6370954">lower back</a>. This is thought to be one of the body’s most sensitive regions to neuroimmune threats.</p> <h2>Immunology basics</h2> <p>Our immune system is a double-edged sword. Yes, it fights off infections for us – but it also makes us acutely aware of the job it is doing.</p> <p>When our body detects an infection, our immune system releases molecules including signalling proteins called <a href="https://www.researchgate.net/publication/227831648_The_functions_of_cytokines_and_their_uses_in_toxicology">cytokines</a>. These proteins coordinate our immune system to fight off the infection and talk to our brain and spinal cord to <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2740752/#:%7E:text=Production%20of%20proinflammatory%20cytokines%20induces,to%20depression%20in%20vulnerable%20individuals.">change our behaviour</a> and physiology.</p> <p>This can result in symptoms like fatigue, loss of appetite, fever and increased sensitivity to pain. Classically, we think of this as a beneficial behavioural change to help us conserve energy to fight off the infection. It’s why we often feel the need to rest and withdraw from our usual activities when we’re sick – and also why we are grumpier than usual.</p> <h2>Invisibly small changes</h2> <p>Part of this self-protective response is a change in how we perceive threats, including sensory stimuli.</p> <p>When we are sick, touch can become painful and muscles can ache. Many changes in behaviour and sensory systems are <a href="https://doi.org/10.1159/000521476">believed</a> to have origins at the nanoscale. When molecular changes occur in part of the brain linked to cognition or mood, we think and feel differently. If these neuroimmune synapse changes happen in the sensory processing regions of the brain and spinal cord, we feel more pain.</p> <p>Such sensory changes, known as <a href="https://www.iasp-pain.org/resources/fact-sheets/allodynia-and-hyperalgesia-in-neuropathic-pain/#:%7E:text=Allodynia%20is%20pain%20due%20to,stimulus%20that%20normally%20provokes%20pain.">allodynia and hyperalgesia</a>, can lead to heightened pain sensitivity, even in areas not directly affected by the infection – <a href="https://www.sciencedirect.com/science/article/abs/pii/S0889159114001731?via%3Dihub">such as the lower back</a>.</p> <h2>Immune memories</h2> <p>This immune response happens with a range of bacterial infections and viruses like COVID or the flu. In fact, the sick feeling we sometimes get after a vaccination is the good work our immune system is doing to contribute to <a href="https://theconversation.com/you-cant-get-influenza-from-a-flu-shot-heres-how-it-works-118916">a protective immune memory</a>.</p> <p>Some of that immune-cellular conversation also alerts our brains that we are sick, or makes us think we are.</p> <p>After some viral infections, the sick feeling persists longer than the virus. We are seeing a long-term response to COVID in some people, termed <a href="https://theconversation.com/when-does-covid-become-long-covid-and-whats-happening-in-the-body-when-symptoms-persist-heres-what-weve-learnt-so-far-188976">long COVID</a>.</p> <p>Women, who generally have a <a href="https://www.nature.com/articles/nri.2016.90">stronger immune response</a> than men, may be more likely to <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7937378/">experience pain symptoms</a>. Their heightened immune response (while beneficial in resisting infections) also predisposes women to a higher risk of inflammatory conditions like <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7980266/">autoimmune diseases</a>.</p> <h2>When to worry and what to do</h2> <p>If the pain is severe, persistent, or accompanied by other concerning symptoms, seek medical attention. Mild to moderate pain is a common symptom during illness and we often notice this in the lower back. The good news is it usually subsides as the infection clears and the sickness resolves.</p> <p>While treating the underlying infection is crucial, there are also ways to dial down sickness-induced neuroimmune pain.</p> <p>Maintaining a diverse microbiome (the collection of microorganisms living in and on your body) by <a href="https://pubmed.ncbi.nlm.nih.gov/31704402/">eating well and getting outside</a> can help. Getting quality sleep, staying hydrated and minimising inflammation <a href="https://karger.com/bbe/article/97/3-4/197/821576/Sickness-and-the-Social-Brain-How-the-Immune">helps too</a>.</p> <p>Amazingly, there is <a href="https://pubmed.ncbi.nlm.nih.gov/34404209/">research</a> suggesting your grandmother’s traditional chicken broth recipe decreases the immune signals at the neuroimmune synapse.</p> <p>Scientists are also <a href="https://pubmed.ncbi.nlm.nih.gov/24799686/">showing</a> mindfulness meditation, cold water therapy and controlled breathing can drive profound cellular and molecular changes to help activate bodily systems like the autonomic nervous system and alter the immune response. These practices might not only help manage pain but also add an anti-inflammatory component to the immune response, reducing the severity and duration of sickness.</p> <p>Heat treatment (with a pack or hot water bottle) might <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8401625/">provide some relief</a> due to increased circulation. Over-the-counter pain relief maybe also be helpful but seek advice if you are taking other medications.</p> <h2>All in the mind?</h2> <p>Is this all mind over matter? A little of yes and a lot of no.</p> <p>The little of yes comes from <a href="https://pubmed.ncbi.nlm.nih.gov/26194270/">research</a> supporting the idea that if you expect your breathing, meditation and cold bath therapy to work, it may well make a difference at the cellular and molecular level.</p> <p>But by understanding the mechanisms of back pain during illness and by using some simple strategies, there is hope to manage this pain effectively. Always remember to seek medical help if your symptoms are severe or persist longer than expected.
